This rewarding role involves providing comprehensive assistance to clients in their homes, fostering independence, safety, and well-being. Duties Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and personal hygiene to promote dignity and comfort. Monitor patients' health status by observing vital signs, recognizing changes, and reporting findings accurately to healthcare professionals. Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary. Support clients with mobility and transfers using proper techniques such as Hoyer lifts or patient transfer methods to ensure safety for both caregiver and client. Administer medications following prescribed care plans while adhering to HIPAA regulations for confidentiality and privacy. Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning, laundry, and organizing living spaces. Provide companionship and emotional support, engaging clients in meaningful activities that enhance mental well-being, especially for those with dementia or Alzheimer's. Assist with transportation for medical appointments or community outings, ensuring safe and timely arrival. Document care activities accurately in client records and communicate effectively with healthcare teams regarding patient progress or concerns. Qualifications Proven experience working with individuals with disabilities, neurodevelopmental disorders, or in senior/geriatric care environments such as nursing homes or assisted living facilities. Knowledge of patient observation techniques, medication administration, first aid, CPR, and basic life support (BLS). Familiarity with specialized care such as Alzheimer's care, memory care, hospice care, and dementia support strategies. Ability to perform manual handling tasks including heavy lifting and patient transfers safely. Strong communication skills for effective client interaction and team collaboration. Experience working with individuals with autism or developmental disabilities is highly desirable. Valid certifications in first aid and CPR; additional certifications related to home health or hospice are a plus. Ability to work independently while following established care plans and protocols in home & community settings.
