Phoenix Senior Living, a leader in engaging digital experiences, is seeking a Resident Care Associate/CNA to deliver exceptional hands-on care in our senior living community. In this onsite role, you will provide personal care, assist with bathing, grooming, mobility, meals, and daily activities while preserving each resident's dignity and independence. You will closely observe and report changes in condition, document care accurately, and partner with nurses, families, and interdisciplinary teams. Responsibilities Provide direct resident care including bathing, grooming, toileting, and mobility support Assist residents with daily activities while promoting dignity and independence Monitor and promptly report changes in residents' physical or mental status Accurately document care, vitals, and observations in electronic records Support medication reminders per community protocols (non-licensed tasks only) Coordinate with nurses, care teams, and families to support care plans Follow infection control, safety, and privacy policies at all times Assist with meals, hydration, and nutritional monitoring Participate in care conferences, in service training, and skill development Contribute to a positive, engaging community environment for residents Required Skills Resident personal care (ADLs/IADLs) Vital signs measurement and monitoring Electronic health record (EHR) documentation Safe transfer and mobility techniques Infection control and standard precautions Dementia and memory care techniques Observation and incident reporting Basic medical terminology Crisis de-escalation and safety awareness Team-based care coordination
