Full-time Job Description Are you a highly organized, compassionate professional looking to make a meaningful difference in your community? We are seeking a dedicated Service Coordinator to join our home health care team in Lawrence, MA. In this vital role, you will serve as the primary bridge between our clients, their families, and our care team. You will be responsible for coordinating day-to-day care schedules, managing client intakes, and ensuring the seamless delivery of high-quality home health services in compliance with state regulations.
Key Responsibilities Care Coordination & Scheduling:
Effectively match and schedule home health aides, nurses, and therapists with clients based on care plans, geographic proximity, and language preferences.
Client Intake & Onboarding:
Act as the main point of contact for new referrals, managing intake documentation and coordinating initial assessments.
Communication Hub:
Maintain regular communication with clients, family members, case managers, and field staff to ensure satisfaction and continuity of care.
Compliance & Documentation:
Maintain accurate, up-to-date electronic health records (EHR) and documentation in compliance with Massachusetts home health regulations and agency policies.
Issue Resolution:
Proactively resolve scheduling conflicts, caregiver call-outs, and client concerns with urgency and empathy.
Collaboration:
Partner with clinical supervisors and HR to monitor field staff performance and assist with caregiver onboarding or retention efforts.
Qualifications & Skills Experience:
1-2 years of experience in scheduling, coordination, or customer service, preferably within a home health, healthcare, or managed care environment.
Education:
High school diploma or equivalent required; Associate's or Bachelor's degree in healthcare administration, social work, or a related field is a plus.
Language Skills:
Bilingual (English/Spanish) is highly preferred to best serve the Lawrence community.
Tech-Savvy:
Proficiency with home care scheduling software (e.g., HHAeXchange, AlayaCare, ClearCare) and Microsoft Office Suite.
