Duties Provide personalized care to clients by assisting with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting Monitor patients' health status through vital signs assessment, patient observation, and reporting changes to healthcare professionals Support residents' mobility using manual handling techniques and patient transfer methods like Hoyer lifts and other assistive devices Prepare nutritious meals and assist with meal planning in accordance with care plans and dietary restrictions Maintain a clean and safe environment by performing light housekeeping tasks, including cleaning patient areas and ensuring infection control standards are met Administer medications following prescribed instructions while adhering to HIPAA regulations for patient privacy and confidentiality Assist with patient transportation and accompany clients to appointments or community activities using personal transportation assistance Requirements Proven experience working as a Personal Support Worker or in similar caregiving roles within group homes, nursing homes, assisted living facilities, or home health environments Knowledge of dementia care, Alzheimer's care, memory care, hospice care, and neurodevelopmental disorders such as autism or intellectual disabilities support Ability to perform patient monitoring, observe for signs of distress or changes in condition, and document findings accurately Familiarity with care plans, medication administration protocols, first aid procedures, CPR certification, and manual handling techniques including heavy lifting and patient transfer methods Strong communication skills to effectively interact with clients, families, and healthcare teams while respecting residents' rights and dignity Experience working within HIPAA guidelines to ensure confidentiality of sensitive health information Flexibility to work various shifts including evenings, weekends, or overnight as required for 24/7 client support Our team is dedicated to providing exceptional home & community care that promotes independence and enhances quality of life.
