Full Job Description Pay:
$15.00 per hour
Schedule:
20 Hrs per week 5 daily Monday - Tuesday 10:00 AM - 3:00 PM Job Summary We are seeking compassionate and dedicated Direct Care Workers / Caregivers to provide high-quality support and assistance to individuals with diverse needs in various care settings. This role involves helping residents with daily activities, monitoring health and safety, and promoting independence and well-being. Ideal candidates are patient, attentive, and committed to delivering respectful, person-centered care in home, community, or long-term care environments. Responsibilities Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Provide medication administration and monitor vital signs in accordance with care plans and HIPAA regulations Support residents with meal preparation, feeding, and nutritional needs while maintaining a clean environment Observe and document patient conditions, behaviors, and changes in health status for care team review Utilize assistive devices such as Hoyer lifts and resident lifts to ensure safe transfers and mobility assistance Maintain accurate documentation of care provided, adhering to HIPAA guidelines Implement individualized care plans tailored to each resident's needs, including dementia, Alzheimer's, or developmental disabilities Support residents' emotional well-being through companionship and engaging activities Collaborate with healthcare professionals to ensure comprehensive patient care across settings such as assisted living, nursing homes, or home health Skills Proven experience in group homes, assisted living facilities, nursing homes, or home & community care environments Knowledge of patient observation techniques and monitoring vital signs Familiarity with Alzheimer's care, dementia care, memory care, and working with individuals with disabilities or developmental disorders such as autism Ability to perform heavy lifting safely using equipment like Hoyer lifts when required Strong understanding of HIPAA compliance and patient confidentiality standards Excellent communication skills for working effectively with patients, families, and healthcare teams Compassionate attitude with patience for working with elderly or disabled populations Experience in meal preparation, cleaning, and maintaining a safe environment for residents This role offers an opportunity to make a meaningful difference in the lives of individuals requiring support.
