We serve victims of domestic violence, sexual assault, human trafficking, child abuse, and other trauma through shelter, mental health services, and comprehensive support services. Summary:
We are looking for a compassionate and organized Case Manager. The case manager is a grant funded position that works primarily with clients that are low-income and in crisis or emergency situations. The case manager is responsible for providing comprehensive case management services to families with children ages 0-5 to promote early childhood development, family stability, and access to essential resources. Role Assignment This assignment will involve working with 0-5 families and individuals seeking services in Tulare County. The Case Manager will provide support and guidance, including working with clients to gather required documentation, conduct intakes, handle enrollments, manage cases, perform home visits, and maintain records. Additionally, the role includes making resource referrals to partner agencies and other necessary services. This position will require some travel throughout Tulare County and occasionally outside Tulare County.
Primary Responsibilities:
Conducts phone intake assessments and in-person appointments; completes client enrollment and eligibility processes and maintains accurate and timely documentation in the case management software and organizational records. Provides comprehensive case management services to families with children ages 0-5, including assessing client needs, developing and monitoring service plan goals, providing guidance and support, and helping families develop life skills and achieve greater financial and family stability. Advocates for clients and families by identifying barriers, coordinating services, and connecting clients with appropriate community resources, emergency assistance providers, and partner agencies. Maintains current knowledge of community resources, eligibility requirements, and referral procedures to ensure families receive appropriate and timely services. Conducts regular home visits to assess client progress, provide support, and promote client and family well-being. Maintains accurate and complete client records, including case notes, service plans, correspondence, notifications, and other required documentation, in accordance with program, grant, HIPAA, and confidentiality requirements. Collects and maintains accurate 0-5 program data to support monthly, quarterly, and other required grant reports. Ensures all required forms, releases, and supporting documentation are completed accurately and submitted in a timely manner. Develops and facilitates a peer-driven parent support group and coordinates Parenting Wisely classes. Facilitates a minimum of 12 workshops annually on topics such as employment, childcare, substance abuse, financial stability, parenting, and other resources available through the Family Resource Center. Maintains and utilizes the Tulare County Family Resource Center Network to strengthen referrals, partnerships, and access to services for participating families. Works collaboratively with community partners and Goshen Elementary School staff to coordinate services, address family needs, and maximize available resources. Participates in and supports Goshen Family Center events, activities, and community outreach efforts. Responds to organizational communications, including voicemail, email, Teams, and text messages, within 24 hours or within the timeframe established by the program. Demonstrates compassion, professionalism, and cultural sensitivity when working with clients and families, particularly those experiencing complex circumstances or crisis situations. Orders, distributes, and maintains program supplies, vouchers, and other approved goods or services, ensuring appropriate inventory and documentation. Participates in required meetings, ongoing training, professional development, and other activities designed to maintain and enhance professional competency. Adheres to all agency policies and applicable requirements regarding HIPAA, confidentiality, information sharing, documentation, and appropriate use of agency client databases and technology. Performs other duties and assumes additional responsibilities as assigned by the supervisor or Leadership Team. Duties may be modified or expanded based on program and organizational needs.
Qualifications:
Associates degree from an accredited college or university with emphasis in social work, psychology, sociology, human services, or a related field. Background check required. Two years of case management or working/volunteer direct service experience may be accepted in lieu of a degree. Excellent written and communication skills. Must have experience with Microsoft Office Suite & ability to work on online platforms. Must demonstrate compassion, especially when clients are experiencing complex issues or in crisis. Ability to demonstrate cultural competence, to understand and value clients' unique values and perspectives. Ability to work both independently and collaboratively in a fast-paced environment. California Driver's License, availability of automobile for business use, and at least minimum auto liability insurance as required by law. Work Environment & Physical Requirements Work is performed in office, visitation center, and/or community-based settings. Position requires sitting, standing, and walking for extended periods. Must be able to lift, push, and pull up to 40 pounds regularly. May require bending, reaching, and repetitive motions (typing, writing, handling visitation materials). May involve exposure to emotionally charged or stressful situations. Occasional evenings or weekends may be required to accommodate event schedules.
