Position Type:

Full Time, In-Person Position Overview Finally Family Homes is seeking a Case Manager to provide direct, individualized support to young adults ages 18-26 who are aging out of foster care, at risk of homelessness, or struggling to find stability. This is a highly relational, in-person role based at our Oasis Resource Center. The Case Manager / Youth Advocate will provide case management and housing navigation while also serving as an active, approachable presence in the Center. This position requires regular engagement with young adults throughout the day, not only during scheduled appointments. The ideal candidate builds trust easily, follows through consistently, and understands the challenges facing young adults experiencing housing instability. Experience with homelessness services, housing navigation, and Los Angeles County resources is especially valuable. Key Responsibilities Provide individualized, client-centered case management services and build consistent, trust-based relationships with young adults Actively engage with young adults using the Oasis Resource Center and help identify needs, goals, and barriers to stability Conduct intake interviews, program orientations, needs assessments, and housing assessments using motivational interviewing and strengths-based approaches Develop and implement short-term and long-term service plans aligned with participant goals Provide trauma-informed support, crisis intervention, conflict resolution, and harm-reduction planning as needed Provide housing navigation , including helping participants identify appropriate housing options, complete applications, gather documentation, and address barriers to securing housing Assist participants with accessing rental assistance, affordable housing, transitional housing, supportive housing, shared housing, and other available housing resources Maintain working knowledge of housing and homelessness resources throughout Los Angeles County Coordinate care and connect participants to resources including housing, food, income, employment, education, healthcare, mental health services, transportation, public benefits, and other basic needs Collaborate with outside agencies and community partners to coordinate services and support participant success Advocate for participants and follow up on referrals, applications, and service-plan goals Maintain accurate and timely documentation, including case notes, participant files, database entries, and reporting requirements Support workshops, groups, events, meals, and community-building activities at the Oasis Resource Center Help maintain a welcoming, safe, respectful, and supportive Resource Center environment Collaborate with Program Managers and other staff regarding participant needs, concerns, progress, and barriers Participate in staff meetings, planning meetings, trainings, and program development Represent Finally Family Homes in community partnerships, collaborative networks, and related events Maintain confidentiality and follow agency policies, procedures, and program requirements Qualifications Bachelor's degree in social services, human services, or related field or equivalent experience Minimum 3-5 years of relevant experience building relationships with young adults facing housing instability, homelessness, or similar challenges Experience with housing navigation , including helping participants identify housing options, complete applications, and overcome barriers to housing Experience navigating community resources and coordinating services Experience with homelessness services and housing resources in Los Angeles County preferred Experience working in collaborative, team-based environments Strong communication, organization, and problem-solving skills Demonstrated commitment to serving young adults aging out of foster care, at risk of homelessness, or struggling to find stability High level of integrity, adaptability, and alignment with the organization's mission Valid driver's license and reliable transportation

Pay:

$27.00 per hour
