PrideStaff is partnering with a highly respected care management and senior advisory organization in the Tampa Bay area to find a dedicated Case Manager. This organization specializes in providing holistic, client-centered guidance to families navigating the complexities of aging, chronic illness, and long-term care. In this role, you will serve as a vital advocate and navigator for older adults and disabled individuals. You will empower families to make informed decisions, help reduce hospital readmissions, and ensure clients maintain the highest possible quality of life. Key Responsibilities Conduct comprehensive in-home or on-site assessments to evaluate the physical, emotional, and safety needs of clients. Design, implement, and continuously monitor individualized care plans that honor the client's personal values, wishes, and goals for independence. Act as the primary liaison between clients, families, medical professionals, and legal/financial advisors to ensure cohesive care delivery. Guide families through transitional care planning, including evaluating readiness for assisted living, coordinating facility moves, or arranging comprehensive in-home support. Oversee the intake process for new clients, performing risk and safety evaluations. Mentor and provide orientation to in-home caregivers, ensuring they are fully equipped to execute the individualized care plans effectively. Identify and recommend community resources, specialist referrals, and asset conservation strategies to families. Maintain compliant, timely, and accurate documentation of all client interactions and care plan adjustments. Act as an ambassador for the organization in the community, occasionally assisting with outreach and educational efforts to cultivate new client relationships. Qualifications Degree or relevant licensure in Nursing (RN/LPN), Social Work (BSW/MSW), Gerontology, Human Services, or a related field. Direct professional experience in geriatric care, case management, home health, or senior advocacy. Thorough understanding of the aging process, chronic disease management, and family dynamics related to eldercare. Working knowledge of local healthcare systems, senior living options, and community resources. Exceptional communication and mediation skills, with the ability to navigate complex family conversations with empathy and objectivity. Highly organized with the ability to manage multiple cases and prioritize tasks independently. Valid driver's license and reliable transportation for visiting clients in their homes or facilities.
