Under the direct supervision of the Program Director, provides direct crisis intervention and stabilization services to clients in the Licensed Crisis Residential Services (LCRS). Ensures the client is stabilized and connected to a case manager upon discharge.
ESSENTIAL DUTIES AND RESPONSIBILITIES
: Facilitates admissions from the Crisis Management Outreach worker into LCRS. Prepares relevant documentation for extended stays in coordination with nurses and clinical supervisors and as necessary, CMO or CMS workers. Documents in the LCRS program Log Book all new admissions and completes all LCRS paperwork including screenings, and discharge and recommendations reports. Conducts and documents assessments. Screens for substance abuse problems. Assesses need for psychiatric or psychological follow up assessments. Coordinates with Crisis Management Team on discharge and disposition. Coordinates with nurse on duty and participates in change of shift report daily. Provides counseling focused on crisis intervention and stabilization for clients. Assists in planning and supervising schedule of daily activities for LCRS program. Develops initial and on-going recovery and treatment plans. Facilitates and documents client progress including counseling sessions, medical information, referrals, general progress, and supervision.
CERTIFICATE/LICENSE/REGISTRATION
CPR/FA Certification Current driver's license, clean driving abstract and current car insurance
EDUCATION AND/OR EXPERIENCE
Bachelor's degree with a minimum of twelve (12) semester credit hours in counseling, criminal justice, human services, psychology, social work, social welfare, sociology or other related behavioral health field. One and one half (1½) years of specialized experience in above field. One to two years of experience working in substance abuse treatment centers preferred.
