Job Description

Seniors Program Case Manager The City of College Park, Maryland, Department of Youth, Family, and Senior Services is seeking a qualified and compassionate Full-Time Case Manager to support the Seniors Program. This position is ideal for a professional who is committed to serving older adults (age 62+) and who brings strong interpersonal, advocacy, and teamwork skills. Position Overview The Case Manager provides individualized support services to older adults to promote safe, independent living. Responsibilities include conducting comprehensive needs assessments, collaborating with community partners, and coordinating and monitoring services based on each client's needs. The Case Manager will have a caseload of up to 30 individuals and provide monthly follow-ups. This is a 40-hour-per-week position, primarily based in the College Park office, with periodic home visits as required. Key Responsibilities Conduct initial and ongoing client assessments Coordinate services, make referrals and connect clients with community resources Advocate for clients to ensure access to appropriate services and support Maintain accurate and timely case documentation Collaborate with internal staff, service providers, and community partners Minimum Qualifications Bachelor's degree in Social Work or a related field (preferred) Experience or familiarity with gerontology and working with older adults Valid driver's license in the state of residence with a satisfactory driving record Strong written and verbal communication skills High level of computer proficiency, including Microsoft Office applications Compensation and Benefits The starting salary range is $33.40- $34.23 per hour, depending on qualifications, along with a competitive benefits package that includes participation in the Maryland State Retirement system.
