Case Manager/Supports Coordinator Assistant

Job Description

A full-time opportunity is available for an individual to join our team and provide case management services to individuals with intellectual and/or developmental disabilities. Responsibilities for a Case Manager or Supports Coordinator include developing individual plans of service linking to appropriate treatment and support services, coordinating care with other providers, and monitoring ongoing progress. Responsibilities include assisting individuals to design and implement strategies for obtaining services and supports that are goal-oriented and individualized. Services include assessment, planning, linkage, advocacy, coordination and monitoring to assist individuals in gaining access to needed health and dental services, financial assistance, housing, employment, education, social services, and other services and natural supports developed through the person-centered planning process. The successful candidate shall possess a Bachelors Degree in Social Work with State of Michigan licensure as a Bachelor's Social Worker (LBSW) or Limited Licensed Bachelor's Social Worker (LLBSW) or be a human services professional with at least a bachelor's degree in a human services related field. A current driver's license and access to an insured vehicle is required. At least one year of experience working with individuals with intellectual/developmental disabilities, adults with mental illness, children with serious emotional disturbance and/or co-occurring disorders is preferred.
