We are currently seeking a Case Manager to join our compassionate and dedicated team to help meet the growing need for high-quality care in our community. Key Responsibilities:

Conduct daily wellness checks to monitor residents' overall health and condition Ensure clear and effective communication among staff to maintain high-quality care Participate in staff meetings and assist with scheduling when necessary Act as a liaison between residents and external parties including doctors, healthcare providers, families, and social services Address and help resolve resident concerns and needs Keep detailed records of notable changes or incidents involving residents Review medical appointment documentation following residents' return from visits Support the medical team during on-site visits from physicians and psychiatrists Maintain accurate care management records, including identified issues and the actions taken Report and document all incidents involving residents, staff, or visitors Monitor the effectiveness of current care plans and ensure proper follow-through and documentation of interventions Quality Assurance - evaluate all current intervention and make sure that such intervention measures of care are closely monitored and documented.

Qualifications:

Strong communication skills with a respectful, dignified approach Ability to build positive relationships with residents, families, visitors, and team members Physical and emotional stamina to support residents in their daily routines Comfortable with technology and proficient in Microsoft Word Physically capable of handling a role with frequent movement and activity Must be at least 18 years of age. Experience in Case Management/Social Services/LPN services. Prefer previous experience, either professional, personal or volunteer, with the elderly population. Must be proficient in computers including Microsoft Word.

Compensation:

$23.00-$26.00/
