Case Manager I | Remote with Ohio Field Visits | Contract Walker Healthforce is seeking a Case Manager I with at least one year of Case Management or Managed Care experience to support individuals with disabilities, chronic conditions, and home care needs. This is a Contract opportunity.

Ross, Vinton, Highland, Pike, Jackson, Gallia, Brown, Adams, Scioto, and Lawrence Counties, Ohio Approved Work Location:
Ohio—candidates must live within reasonable traveling distance of the assigned service counties
Hours/Schedule:
Monday-Friday, 8:00 AM-5:00 PM EST; approximately 0-3 home visits per week based on member needs
Worker Type:
W2 Core Requirements Active, unrestricted Ohio RN, LSW, or LISW license Minimum one year of Case Management or Managed Care experience Minimum one year of experience working with individuals with disabilities, chronic conditions, or home care supports Experience conducting member assessments Experience developing and maintaining individualized care plans Experience authorizing or coordinating healthcare services Ability to work independently in remote and community-based settings Strong organization, communication, and problem-solving skills Valid driver's license with a good driving record Reliable transportation and ability to travel throughout the assigned Ohio counties We Consider It A Bonus If You Also Have Home health or home care experience Community-based Case Management experience Medicare or Medicaid experience Experience coordinating behavioral health or long-term care services Experience participating in Interdisciplinary Care Team meetings Interest in future full-time conversion Job Responsibilities Conduct telephonic outreach and home visits with members receiving home care services Assess member healthcare and support needs Authorize appropriate services Develop and maintain individualized care plans Monitor service delivery and member progress Coordinate care with members, caregivers, providers, and community resources Modify care plans when member needs change Support members with disabilities and chronic health conditions Document assessments, interventions, and care plan activity Participate in interdisciplinary care planning Complete approximately 0-3 home visits per week based on member needs Maintain productivity, quality, confidentiality, and regulatory compliance What We Offer Competitive compensation package Weekly pay via Direct Deposit Medical, Dental & Vision available 401k options Why Work With Walker?
