Waiver Service Coordinator

Job Description

Direction Home Akron Canton is seeking a dedicated and compassionate Waiver Service Coordinator to help older adults and individuals with disabilities remain safe, healthy, and independent in their homes. Key Responsibilities Conduct in person home assessments to determine eligibility, care needs, and service options. Complete home visits in Summit, Stark, Wayne and Portage County as well as phone contacts to monitor member health, safety, and satisfaction. Develop and maintain individualized Waiver Service Plans. Coordinate services and collaborate with healthcare providers, caregivers, and community partners. Assist members with Medicaid eligibility and access to community resources. Provide education, advocacy, and support to members and their families. Respond to member needs, critical incidents, and service concerns while maintaining accurate documentation. Qualifications Required:
One of the following: Registered Nurse (RN) licensed in the State of Ohio Licensed Social Worker (LSW) licensed in the State of Ohio Licensed Practical Nurse (LPN) licensed in the State of Ohio Social Work Trainee with approved Ohio registration Bachelor's degree in a field related to serving older adults and individuals with disabilities, with qualifying experience as defined by
OAC 174-43-05
(B)(2)(d) Minimum of one year of experience working with individuals with disabilities, chronic conditions, or those requiring long-term services and supports (LTSS)
Preferred:
Previous care management or care coordination experience One year of experience in home care, geriatrics, behavioral health, or long-term care Knowledge of chronic disease management
