About Us Jordan CRC and Phoenix Counseling Solutions provide behavioral health, substance use treatment, recovery housing, case management, and employment support through our Individual Placement and Support (IPS) and Opportunities for Ohioans with Disabilities (OOD) programs. We serve women working toward recovery and long-term stability by providing coordinated, trauma-informed, and evidence-based care. We serve women experiencing substance use disorders, mental health concerns, trauma, homelessness, criminal justice involvement, and other barriers to recovery. Position Summary Serves as the primary counselor for an assigned caseload of clients receiving behavioral health and substance use treatment services. This position provides individual counseling, group facilitation, case management, treatment planning, and care coordination while supporting clients from admission through discharge. Responsibilities Conduct assessments and reassessments within scope of licensure. Develop and update individualized treatment plans. Provide individual counseling services. Facilitate therapeutic and psychoeducational groups. Maintain an assigned caseload and monitor client progress. Coordinate services with courts, probation officers, healthcare providers, community agencies, and other referral sources. Assist clients with housing, benefits, recovery supports, and community resources. Participate in treatment team meetings and case reviews. Collaborate with the agency's IPS and OOD programs by incorporating employment and vocational goals into treatment planning, progress notes, and care coordination when clinically appropriate. Complete discharge planning and discharge documentation. Maintain accurate and timely documentation in the electronic health record. Ensure compliance with Medicaid, OhioMHAS, Joint Commission, and agency requirements. Qualifications Required Ohio Licensed Social Worker (LSW) Bachelor's degree in social work Valid driver's license and reliable transportation Access to reliable transportation Cleared criminal background check Cleared drug screen Preferred Master's degree in social work Experience working in behavioral health, substance use treatment, recovery housing, or community mental health Experience working with trauma, co-occurring disorders, and justice-involved populations Schedule Weekdays 6-hour, 8-hour and 10-hour shifts available Why Join Our Team?
