The incumbent will also provide professional assessment, service planning, service coordination, referral, and re-evaluation services required for a safe and healthy community life, which is manifested through providing and coordinating care by stability in relationships, housing, and employment to assist in the recovery process.

Minimum Qualifications Education:

Bachelor's degree with major course work (min. 12 credit hours) in social work, psychology, criminal justice, pastoral counseling, counseling psychology, rehabilitation counseling, and sociology or education with a certificate in special education or nursing with a Registered Nurse (RN) License.

Experience:

Minimum of 1 year of experience in human services and working knowledge of human services paperwork and regulations.

Licenses, Certifications & Clearances:

• Act 33/34 Clearances
• FBI Clearance
• Valid State of PA Class 1 Drivers License
• Access to properly insured motor vehicle for client transportation and fieldwork and a safe driving record

• Knowledge about convening and facilitating service planning meetings.
• Knowledge of child abuse reporting.
• Knowledge of the role of advocacy as a central service coordination function and on service planning.
• Knowledge of the theories and principles used in providing Service Coordination services.
• Knowledge of position responsibilities, including state mandates and county expectations.
• Knowledge of systems theory, family systems, family therapy interventions, and psychoeducation.
• Ability to identify and manage the symptoms of severe mental illness.
• Knowledge of medications for managing symptoms of the symptoms of severe mental illness.
• Ability to broker resources.
• Ability to manage crises.
• Ability to work professionally and effectively with other community agencies, the general public, and agency staff.
• Knowledge about identifying and managing substance use problems.
• Knowledge of 12-step programs, groups, area resources, recovery/prevention/rehabilitation, child and adolescents with disorders, planning, and documentation.
• Ability to identify symptoms, support systems, and community resources.
• Knowledge of the treatment needs of MISA population, as well as internal and external resources.
• Ability to work with children and adolescents who are in placement, receive services of FSS (Financial Support Services), or who need coordination of care following an inpatient stay.
• Knowledge of the needs of this population and community resources (inpatient units, dual diagnosis, intensive outpatient services, etc.).
• Knowledge about identifying and managing co-morbid health problems and promoting wellness.
• Ability to help individuals find meaningful activity, employment, and maintain employment.
• Knowledge of the guidelines and regulations for all levels of Service Coordination. Essential Duties & Responsibilities
• Possesses orientation of systems theory, family systems, family therapy interventions, and psychoeducation.
• Establishes a collaborative service plan using processes.
• Acts as Convener/Facilitator in service planning across programs and agencies.
• Develops Crisis Plans and WRAP Plans.
• Develops mental health advanced directives.
• Coordinates services across programs/providers.
• Problem Resolution - advocacy, data, grievances.
• Recovery Resources - informal supports, services.
• Familiar with dual diagnosis (mental illness/substance use or addiction).
