Youth Case Manager Tri-Starr Talent San Antonio, TX Job Details Full-time $46,000 - $49,000 a year 18 hours ago Qualifications Case assessment Bachelor's degree Family communication in social service case management Social work care coordination Full Job Description The Youth Case Manager supports children in conservatorship by coordinating services, managing case plans, and working toward permanency outcomes such as reunification, kinship placement, or adoption. This position works closely with families, caregivers, and community partners to help ensure children are placed in safe, stable, and supportive environments.
Work Location:
Field-Based Position - San Antonio and surrounding areas Key Responsibilities Manage a caseload of children in care and support permanency planning efforts. Assess child safety, family needs, and placement options. Develop and implement service and permanency plans supporting reunification, kinship care, or adoption. Conduct home visits, family assessments, and meetings with families and service partners. Maintain accurate case notes, reports, and documentation in accordance with program requirements. Coordinate services with internal teams, courts, and community agencies. Provide guidance and ongoing support to children, families, and caregivers throughout the case process. Respond to crisis situations and provide on-call support as needed. Participate in team meetings, required trainings, and program activities. Required Qualifications Associate's degree in Child Welfare or a related field or 60 college credit hours and two years of relevant experience Relevant experience may substitute for education.
Pay:
$46,000.00 - $49,000.00 per year
