Overview The Housing Case Manager works with families and individuals who are homeless, have experienced periods of homelessness, and/or are experiencing significant risks of homelessness. Using a housing-first approach with those individuals being served, the Housing Case Manager focuses efforts on any areas that directly impact clients obtaining and maintaining permanent housing, while also ensuring the appropriate documentation and reports are completed in a thorough and up-to-date manner. The Housing Case Manager is responsible for the planning, coordination and implementation of all housing services offered by STEPS, Inc. This position is supervised by and reports to the Vice President of Empowerment Services. ESSENTIAL FUNCTIONS
- Responsible for assisting with determining client's eligibility for financial assistance as well as monitoring grant compliance and spending.
- Responsible for developing, providing, and monitoring individualized housing action plans with the primary goal of obtaining and maintaining permanent housing. This includes identification of existing barriers to self-sufficiency, setting housing, income goals and developing an action plan in collaboration with other service team members.
- Responsible for working with landlords and tenants to conduct required rental housing inspections that include lead-based paint assessments, in compliance with all applicable program requirements.
- Responsible for attending staff meetings, housing training, and in-service training as needed or necessary to maintain best practice housing approaches / certifications.
- Responsible for providing individual, housing-focused case management for clients, serving as coordinator of services, providing assessment, referral and follow-up to address any needs that impact housing stability.
- Responsible for invoicing, obtaining W9 forms from landlords, and processing documentation for payments.
- Responsible for establishing a good working relationship with other public and private agencies/services, interfacing collaboratively and establishing on-going linkages and support for individual clients.
- Responsible for establishing a professional, supportive relationship with all clients, landlords, Heartland Local Planning Group Board of Directors/members, other community resources and the Re-entry Council.
- Responsible for ensuring that all clients understand program guidelines and remain focused on housing stability goals.
- Responsible for conducting intake interviews including case history and assessment of needs.
- Responsible for educating and assisting households regarding existing community services, and acting as an advocate in connecting individuals to these services.
- Responsible for regularly evaluating activities and accomplishments of clients with relation to their established goals in order to ensure continued program fulfillment.
- Responsible for bi-weekly, post move-in, follow-up meetings with clients for assessment of compliance and to offer tenant support on community-based services.
- Responsible for documenting consistently, completing all relevant forms, including intakes, assessments, case management notes, quarterly and monthly grant reports.
- Responsible for accurately completing all required data collection and reporting in accordance with established guidelines.
- Responsible for participating in all data collection trainings.
- Responsible for participating in development and fundraising activities as needed and requested (ex: meeting with donors, providing tours, attending fundraising events).
- Responsible for ensuring that all clients files are complete, current, accurate, meet the requirements of all monitoring agencies and are securely maintained.
- Responsible for performing other duties as assigned by the Vice President of Empowerment Services.
- Responsible for attending, reporting to, and, at times, hosting Heartland Local Planning Group meetings.
- Responsible for meeting National Alliance to end Homelessness Rapid Re-Housing standards.
- Responsible for back-up supervision and guidance of the Housing Assistant.
QUALIFICATIONS
Graduate from an accredited college/university with a degree in Human Services or related field preferred. Experience working in government funded programs preferred. A combination of education/work experience may be substituted. Must have a valid Virginia Driver's License and acceptable driving record. Must pass drug screening and qualify to pass criminal background check. Must maintain housing certification. Must be willing/able to work weekends, nights and holidays as needed. Must be organized and able to work independently with minimal direct supervision.
