Housing Case Manager Bayside Housing & Services Position Summary:
Bayside Housing Services is seeking a compassionate, organized, and motivated Housing Case Manager to provide person-centered case management services to individuals and families experiencing homelessness. The Housing Case Worker supports residents living in Permanent Supportive Housing, Tiny Home Temporary Housing, and Veteran Housing programs by helping them achieve housing stability, improve self-sufficiency, and access community resources. This position works collaboratively with participants, outside case managers, behavioral health providers, medical providers, Veterans Affairs staff, and community agencies to coordinate services and promote successful housing outcomes. Services are delivered in accordance with Washington State Foundational Community Supports (FCS) guidelines, Medicaid documentation standards, and agency policies when applicable.
Essential Duties and Responsibilities:
Provide individualized, strengths-based case management using trauma-informed and person-centered practices.
Assist participants in obtaining and maintaining stable housing while promoting independence and long-term success.
Develop individualized housing stability and service plans based on participant goals and needs.
Coordinate services with outside case managers, behavioral health providers, medical providers, Veterans Affairs, DSHS, Coordinated Entry, and other community partners.
Conduct regular home visits and wellness checks with residents in Permanent Supportive Housing, Tiny Home Temporary Housing, and Veteran Housing programs.
Assist participants with accessing benefits, employment resources, transportation, healthcare, behavioral health services, food assistance, and other community resources.
Support participants with budgeting, life skills, landlord communication, lease compliance, and conflict resolution.
Advocate on behalf of participants to reduce barriers to housing stability.
Maintain timely, accurate, and complete documentation that meets agency, FCS, Medicaid, and funding source requirements.
Participate in team meetings, interdisciplinary care coordination.
Maintain professional boundaries, participant confidentiality, and compliance with HIPAA regulations.
Respond appropriately to crises and connect participants with emergency or behavioral health resources as needed.
Minimum Qualifications:
Bachelor's degree in Social Work, Human Services, Psychology, Sociology, or a related field; or an Associate degree with relevant experience; or an equivalent combination of education and experience.
Experience working with individuals experiencing homelessness, behavioral health challenges, substance use disorders, veterans, or other vulnerable populations preferred.
Knowledge of Housing First, Harm Reduction, Motivational Interviewing, and Trauma-Informed Care principles preferred.
Strong communication, organization, and documentation skills.
Proficiency with electronic documentation systems and Microsoft Office.
Valid Washington State driver's license, reliable transportation, and proof of insurance. Preferred Qualifications
Experience providing Foundational Community Supports (FCS) Supportive Housing services.
Experience working within Medicaid-funded programs.
Familiarity with local housing resources, Coordinated Entry, landlord engagement, and supportive housing models.
Experience working with veterans and veteran service organizations.
CPR/First Aid certification (or ability to obtain). Knowledge, Skills, and Abilities

Physical Requirements:
Frequent travel throughout the community.
Ability to conduct home visits
Ability to occasionally lift up to 25 pounds.
Ability to work in office, housing sites, and community settings
