Compensation:

$20/hour base + performance-based commission, with earning potential up to approximately $50/hour About Nurra Nurra supports patients with complex healthcare needs through high-touch, compassionate care coordination. We combine human connection with thoughtful systems and technology to help patients feel supported, informed, and cared for. Through our Chronic Care Management (CCM) program, we work alongside patients and their healthcare providers to help patients stay on top of their care, navigate their health needs, and remain connected to their care team. As we expand the Nurra CCM program in Modesto, we're looking for someone local who can help introduce the program to patients, explain its benefits, and make enrollment simple and welcoming. About the Role Nurra is looking for a local Contractor Outreach & Enrollment Coordinator in the Modesto area to support patient outreach and enrollment for our Chronic Care Management (CCM) program. A key focus of this role will be in-person patient engagement at our Modesto and Manteca clinics. You'll serve as an on-the-ground Nurra presence, connecting with eligible patients, explaining the CCM program, answering questions, and helping interested patients enroll. Additional assignments may include remote patient outreach and follow-up by phone and text. This is a flexible, project-based contract opportunity rather than a role with a guaranteed weekly schedule. Hours may vary based on clinic and outreach needs. This role is a great fit for someone who genuinely enjoys talking with people, is confident approaching patients in person, and is motivated by helping patients understand and take advantage of services available to them. What We're Looking For Located in or near Modesto, California, with reliable transportation to the clinics Strong interpersonal skills and confidence approaching and speaking with new people Warm, engaging communication style and the ability to quickly build rapport Comfortable discussing healthcare services with patients in a clear and professional way Comfortable conducting outreach both in person and by phone/text Goal-oriented and motivated by performance-based incentives Highly organized, reliable, and detail-oriented Comfortable working independently and taking initiative Able to work a flexible, as-needed schedule based on clinic and outreach needs Experience in healthcare, patient outreach, community outreach, enrollment, customer service, or sales is a plus, but just as important is the ability to connect with people and make them feel comfortable and supported.
