This position combines direct family support with active participation in community events, outreach initiatives, and service coordination. The Navigator manages a diverse caseload, provides screenings and assessments, assists families in accessing resources, and ensures consistent communication with partner agencies. The ideal candidate will demonstrate strong organizational skills, the ability to manage multiple responsibilities, and a commitment to empowering families through advocacy and support. Community Navigator Responsibilities Conduct intake screenings, including social isolation and mental health screenings, for new and ongoing clients. Maintain and provide weekly and monthly reports on outreach and caseload activities. Assist clients with applications, including: CalWORKs In-Home Supportive Services (IHSS) Inland Regional Center (IRC) SSI, TRIP, and mini-grant applications Serve as support for families during the IEP process by gathering information, providing resources, and assisting them with questions. Provide in-depth case management by connecting families to resources, referrals, and ongoing support. Assist with the coordination and facilitation of socialization groups to help families and children engage with their community. Conduct surveys to gather program impact data and assess client needs. Manage a caseload of moderate size and complexity, normally involving multiple areas of disability support. Interview clients, families, and other responsible individuals; assist in completing service applications; collect and analyze basic data; and obtain additional information from outside agencies. Prepare and maintain case progress notes, social summaries, and related reports. Create and revise systems and procedures by analyzing operating practices, data tracking, and outreach processes. Resolve administrative challenges by coordinating report preparation, analyzing data, and identifying solutions. Provide information and referrals to clients, families, and partner agencies. Represent Angel View Outreach at community events, resource fairs, and collaboration meetings across Riverside County (travel required). Assist in organizing and facilitating outreach events and presentations. Manage outreach efforts using program databases, ensuring accurate tracking and reporting of community engagement. Maintain professional knowledge by attending workshops, building networks, and participating in community outreach events. Contribute to team success by accomplishing related tasks and supporting program goals. Required Skills and Abilities Prior community outreach or social services experience. Ability to carry and manage a heavy caseload. Strong organizational skills with the ability to prioritize and organize workload. Willingness and ability to learn outreach database systems. Strong communication and interpersonal skills to work effectively with diverse populations, cultures, and sub-cultures. Skilled in Excel, Microsoft Word, and general computer use. Ability to work some weekends and evenings (with advance notice). Ability to handle high volumes of phone calls and client interactions. Bilingual English/Spanish required. Must be willing to travel throughout Riverside County (up to 200 miles). Education & Experience Bachelor's degree and at least two years of experience (or equivalent combination of education and experience). Good listening and problem-solving skills. Ability to work both independently and as part of a team. Excellent oral and written communication skills. Must have a current CA driver's license, clean driving record, and reliable insured vehicle. Additional Information This position requires frequent travel, a heavy outreach workload, and a high level of community engagement. Additional responsibilities may be assigned at the supervisor's discretion.
