As a Hospice Outreach Representative for OpusCare of South Florida, you will be a direct presence in the community by providing information and opportunity to potential clients. Responsibilities for this role include planning and directing marketing of company's services through personal effort and through the cooperation of supportive team and management. By establishing and maintaining relationships with hospitals affiliates, care facilities, and in the community, you will promote excellent outcomes for our patients and strategic partners. We seek a proficient and seasoned Representative with excellent work ethic. Personable, driven, organized and well versed in the healthcare industry are mandatory skills in this position. Key Responsibilities Develop and maintain relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, home health agencies, social workers, case managers, clergy, and other referral partners. Educate healthcare professionals, patients, families, and community organizations about hospice services and the benefits of hospice care. Conduct regular outreach visits, presentations, educational programs, and community events. Identify and develop new referral sources and community partnerships. Maintain consistent communication with existing referral partners to strengthen relationships and encourage continued collaboration. Respond promptly to inquiries regarding hospice services and facilitate appropriate referrals. Collaborate with the clinical and admissions teams to ensure a smooth transition from referral to hospice admission. Represent the hospice organization professionally at community events, healthcare meetings, networking functions, and educational programs. Track outreach activities, referrals, leads, and relationship development using the organization's CRM or reporting system. Meet established outreach, referral, and growth goals. Monitor community needs, healthcare trends, and competitor activity and communicate relevant information to leadership. Participate in marketing initiatives, educational campaigns, and promotional activities. Provide compassionate support and appropriate resources to families and community members seeking information about end-of-life care. Maintain compliance with all applicable hospice regulations, organizational policies, and ethical standards. Protect patient and referral-source confidentiality at all times. Meet with Hospice patients and family members to discuss services provided by OpusCare. Required Qualifications High school diploma or equivalent; associate or bachelor's degree in healthcare, business, marketing, communications, social services, or a related field preferred. 2+ years of experience in healthcare outreach, business development, sales, community relations, marketing, hospice, home health, senior care, or a related field preferred. 1-2 years experience in marketing/sales in Hospice, Home Health, Pharmaceutical, Assisted Living Facilities, Skilled Nursing Facilities, or other healthcare sales experience preferred. Strong interpersonal, communication, presentation, and relationship-building skills. Demonstrated ability to establish and maintain professional relationships with healthcare and community partners. Determined, collaborative, dedicated personality a must. Great communication skills and ability to work independently. Great interpersonal skills. Bilingual preferred.
