Job Description

We are seeking a compassionate, personable, and community-focused Community Liaison with a passion for relationship-building, outreach, and serving others. This role is ideal for someone who enjoys connecting with people, representing the agency in the community, developing meaningful partnerships, and helping families navigate available care and support services. Position Overview As the Community Liaison at Angel Caregivers, Inc., you will serve as a key representative of the agency within the community. You will develop and maintain relationships with healthcare professionals, senior living communities, community organizations, families, and other local partners to increase awareness of our services and strengthen connections to individuals who may benefit from in-home care. The Community Liaison will participate in community events, conduct outreach, establish collaborative partnerships, and serve as a trusted resource for individuals, families, and organizations seeking information about caregiving services. Your success in this role will be measured by your ability to build strong community relationships, increase awareness of Angel Caregivers' services, create meaningful partnerships, and consistently represent the agency with professionalism, compassion, and integrity.
Key Responsibilities Drive Community-Based Lead Generation:
Proactively identify and engage referral sources such as hospitals, rehabilitation centers, assisted living facilities, physicians' offices, and community organizations.
Build Referral Pipelines:
Establish, grow, and maintain a strong network of referral partners through regular in-person visits, presentations, follow-ups, and community networking events.
Promote Agency Services:
Clearly communicate the benefits and value of Angel Caregivers' home care services to referral partners, families, and community stakeholders.
Execute Outreach Strategies:
Develop and implement outreach plans that increase brand awareness and generate consistent, high-quality leads.
Create and Use Marketing Materials:
Collaborate with internal teams to develop brochures, presentations, and outreach tools that support sales conversations.
Generate and Qualify Leads:
Actively prospect through cold calling, direct marketing, email outreach, and social media to convert interest into referrals.
Track Sales Performance:
Monitor referral activity, market trends, and competitor presence to identify growth opportunities and improve outreach effectiveness. Collaborate with
Care Coordination:
Ensure smooth transitions for new clients and maintain satisfaction through close collaboration with the care team.
Maintain CRM Records:
Accurately document outreach activities, referral contacts, and lead status in the CRM system.
Meet and Exceed Growth Goals:
Consistently achieve or surpass monthly and quarterly sales and referral targets.
Qualifications Education:
High school diploma or equivalent (preferred) Bachelor's degree in Business, Marketing, Healthcare Administration, or a related field (preferred) 3+ years of experience in community outreach, relationship building, and business development ,preferably within the healthcare or home care sector Strong focus on developing community partnerships, engaging referral sources, and connecting individuals and families with appropriate care services. Confident communicator with strong presentation abilities Self-motivated and comfortable working independently in the field Proficient in Microsoft Office and CRM systems Reliable transportation and a valid Driver's License Join us as a Community Liaison where your energy fuels our growth!
