Job Description

Case Coordinator - Bilingual English/Spanish Growing Homecare company providing services throughout the NYC area is looking for a Bi-Lingual Case Coordinator.

This position will:

Coordinate and Schedule appointments for Community Health Workers (CHW) Ensure all appointments are accurately updated in Master list and corresponding notes are maintained Collect files, assign members, and monitor performance of Community Health Workers (CHW) to ensure they are able to complete their scheduled visits Manage client accounts and maintain accurate records

Requirements:
