Wellness Liaison The Wellness Liaison is responsible for creating an excellent customer experience and will extend the activities of a Manager of Community Partnerships. Some of the key responsibilities will be outreach and engaging prospective participants, conducting comprehensive and accurate intakes onsite at key accounts and providing educational workshops. Position will be responsible for working to identify and assist prospective participants with procuring documentation needed, follow up with intake appointment and follow through at various stages of intake. The Wellness Liaison will continue to maintain contact with the clients until he/she is transitioned to Outreach program/service. The Wellness Liaison is an Outreach Ambassador and will excel in exemplifying the core values. •We are seeking candidates who are familiar with the Far Rockaway, Queens area•.
Essential Duties:
Extend activities of Manager of Community Partnerships in an account by providing intakes and preparing and providing educational workshops. Maintain professional presence inside the assigned referral sources/accounts and provide excellent customer service. Provide onsite screenings/assessments/intake at hospitals, courts, housing sites and other locations as needed. Work with Manager of Community Partnerships to identify potential clients for Outreach programs and services. Conduct comprehensive intake and complete required documentation. Achieve monthly/ quarterly targets for Wellness Liaison. Maintain contact with the referred participant until the admission is complete and there is a warm transition to Outreach program. Act as a liaison between external referral sources and internal Outreach programs to ensure internal customer understands account needs. Work with internal programs to ensure referrals/admissions are expedited; assists in getting any necessary documentation or information needed to process referrals.
Minimum Requirements:
High School Diploma/GED Equivalent required, Bachelor's Degree a plus. 1 -2 years of work experience. Knowledge and experience working with at risk populations a plus
