Community Outreach Coordinator for

Senior Care Job Description:

Devi Adult Day Services is seeking to fill a Community Outreach Coordinator position based out of the Bethlehem location. This Individual will interface with Service coordinators, health care offices and Adult daycare Directors to drive referrals to attend the Senior Adult Day Program. The goal is to increase our brand and drive in client admissions. The candidate must have a network connection of resources in the Lehigh valley/surrounding areas, hospitals, senior housing, etc. Coordinator will devise a marketing plan to perform key marketing functions with the direction of Management. The employee will be trained on various provider / insurance / vendor billing systems to perform client intake & referral / authorization management. The coordinator will be required to work with Case Managers to obtain authorizations for adult daycare and transportation services to ensure eligibility and tracking of missing authorizations. Work with the Veterans Administration, Department of Human Services and Home & Community Based Services programs for Seniors. Candidate must be willing to learn and be trainable and possess a strong work ethic. This individual must be extremely detailed oriented and possess excellent organizational skills and comfortable working in a high paced environment. Candidate will focus on driving in new enrollments by working with the below channels - Families/Caregivers, hospitals and rehab referrals, Area Agency on Aging, Medicaid waiver contacts. Candidate should have working experience / a network of contacts with Physicians and various health care providers. This Coordinator will have experience working local marketing methods as well as online/digital methods through Google, Facebook, etc.

QUALIFICATIONS

Two or more years' experience working in the Human Services field preferred. Must have excellent Customer Service & Communication skills Prior experience working with computer systems, google drive required. This individual shall possess a clean driver's license for at least 3 years and not have been convicted of a crime. (Criminal, DMV Background check, Drug & Alcohol tests are required this position.) Experience working with Geriatric population Computer skills and experience with office management / billing a plus Bi-Lingual / Spanish is a plus.
