Come Work for the Region's Most Trusted Hospice Care Provider! Hospice & Community Care of Rock Hill, SC is actively hiring a Full Time Hospice Outreach & Referral Coordinator! Hospice & Community Care has been a cornerstone of compassionate end-of-life care in our community for four decades. What truly sets us apart is our dedication to our community. We provide charity care for the underinsured, maintain a free medical equipment lending closet, offer grief support, and proudly support our Veterans through a robust program. We are seeking a Hospice Outreach & Referral Coordinator that will meet with patients and families to explain our hospice services and what they can expect from hospice care, work closely with our intake and clinical teams to coordinate referrals, and develop strong relationships with hospitals, physicians, skilled nursing facilities, assisted living communities, and other healthcare partners throughout the Rock Hill region.
What You'll Do:
Patient & Family Outreach Visit patients and families, provide compassionate education about our hospice services, and work closely with intake and admissions to coordinate timely referrals and transitions to care. Referral & Community Outreach Build and maintain relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, and other healthcare partners through outreach visits, education, presentations, and community events. Relationship Development Develop new referral relationships, strengthen existing partnerships, and represent the organization professionally while identifying opportunities to increase awareness and appropriate hospice referrals. A valid driver's license and reliable transportation are required for travel throughout our service territory.
Preferred Qualifications:
Bachelor's degree in Marketing, Business, or a health-related science (e.g. Nursing, Pharmacy or Social Work) Previous experience in healthcare marketing, hospice, palliative care, or home health General understanding of hospice referral partners (hospitals, physician, skilled nursing facilities, and assisted living facilities) General knowledge base of the healthcare providers in our service areas: York, Chester, and Lancaster counties. Strong communication and interpersonal skills Experience developing referral relationships with healthcare providers Proven ability to generate new business and maintain long-term professional relationships.
