Job Description

Addison Housing Works is seeking a full-time SASH Coordinator to help launch and lead our Support and Services at Home (SASH) program in Middlebury. This is a great opportunity for someone who loves working with older adults, builds relationships easily, and wants to make a tangible difference in helping people remain healthy, connected, and independent in their own homes. The SASH Coordinator gets to know participants individually—their strengths, goals, needs, and support networks—and helps connect the pieces that allow them to age successfully at home. This is a varied, people-centered role that combines direct participant support, care coordination, and community-building. You'll work closely with a SASH Wellness Nurse and a team of local health and social-service partners; help participants navigate transitions home from hospitals or rehabilitation facilities; connect people with services and community resources; and coordinate wellness, educational, and social activities. You'll also build strong relationships with local providers, families, volunteers, and community organizations and serve as a central point of communication when participants need additional support. We're looking for someone who is warm, organized, resourceful, and comfortable taking initiative. The right person will be equally comfortable sitting down with an older adult to understand what matters to them, leading a team meeting, organizing a community activity, following up with a healthcare provider, and keeping good records so nothing falls through the cracks. A bachelor's degree in social work or a comparable combination of education and experience is required. Experience working with older adults and/or people with disabilities and familiarity with Addison County resources are strongly preferred. Strong communication skills, sound judgment, comfort with technology, and a valid driver's license are also important. Pay:
$23.00 - $25.00 per hour Expected hours: 40.0 per week
