Job Description

Essential Functions:
Admissions Coordination Gather pre-admission data and interview residents, families, and representatives to collect accurate information, explain the admission process, and determine level of care and the facility's ability to meet the resident's needs. Review referral charts and follow up with referring facilities, agencies, and individuals to obtain the medical, social, and financial information needed to process applications. Explain the admission agreement, facility philosophy, policies, and procedures; complete admission forms accurately and timely; obtain required signatures; and request advance directive information. Conduct on-site nursing assessments at home, hospital, or other facilities, reviewing medical history, medications, and psychosocial needs. Admit residents in a welcoming manner: prepare ID bracelet, face sheet, and photos; code diagnoses; and enter all demographic, insurance, and billing data in the ADL system. Notify departments and families of admissions, discharges, transfers, and room changes, and contact hospital case managers to monitor hospitalized residents. Hospital Liaison / Referral Development Serve as primary liaison to hospitals, physicians, discharge planners, case managers, and community partners to develop and sustain referral relationships supporting consistent census and quality mix. Make calls and visits to referral sources (hospitals, social workers, physicians, discharge planners, home health, clergy, civic and professional organizations, families) to communicate services and secure referrals. Field inquiries, provide facility and admission information, and give tours to prospective residents, families, and referral sources. Analyze discharge planner information to confirm the facility can meet resident needs, advise them of outcomes, and anticipate and resolve barriers to admission. Maintain a referral tracking system to identify trends, build an effective referral plan, and communicate trends to leadership. Attend weekly referral development meetings and daily stand-up and admissions meetings, and participate in creating materials that promote the facility. Resident and Family Experience Interact with residents, families, and representatives daily, respond to their questions, and guide them to the appropriate service. Coordinate across departments to promote teamwork and a seamless resident and family experience. Provide a channel where residents and families can voice issues or complaints without fear that it will compromise their care or future access to care; accept, document, investigate, and resolve complaints and grievances per policy, ensuring proper notifications and interventions. Explain resident rights, responsibilities, and complaint procedures to residents, families, and other involved persons, and refer concerns to the appropriate individuals.
Education:
Associate's degree required, Bachelor's degree preferred
Experience:
Two (2) to five (5) years hospital, healthcare or related experience required; skilled nursing experience preferred.
Certification:
N/A Licensure:
Valid Florida Driver's License required.
Training:
Knowledge of Microsoft applications required.
