Overview We are seeking a dynamic and strategic Director of Outreach to lead our community engagement and external partnership initiatives. The Director of Outreach is responsible for leading Eldercare's outreach, referral development, and admissions efforts across our network of Personal Care Homes throughout Kentucky. This individual will build strong relationships with hospitals, discharge planners, case managers, physicians, behavioral health providers, skilled nursing facilities, community organizations, and other referral partners to generate qualified referrals and increase occupancy. The Director of Outreach will work alongside the admissions team and facility administrators to ensuring every prospective resident and family receives timely, compassionate, and professional service. Essential Responsibilities Outreach & Business Development Develop and maintain referral relationships throughout Kentucky. Serve as the primary representative of Eldercare within the community. Conduct regular outreach visits, presentations, and networking activities. Identify new referral opportunities and implement growth strategies. Promote Eldercare's mission, services, and reputation. Referral & Admissions Management Manage referrals from initial inquiry through admission. Coordinate assessments, tours, and admissions across the Eldercare network. Work closely with facility leadership to identify appropriate placements and available capacity. Maintain timely communication with referral sources, residents, and families. Ensure a positive and responsive admissions experience. Leadership & Performance Track referral activity, admissions, and occupancy trends. Provide regular reports and recommendations to executive leadership. Collaborate with facility leaders to improve referral processes and customer service. Support organizational growth through relationship building, accountability, and operational excellence. Qualifications Three or more years of experience in healthcare outreach, admissions, business development, senior living, or a related field. Understanding of the population we serve and agencies we work with Proven ability to develop referral relationships and increase admissions. Excellent communication, organizational, and relationship-building skills. Valid driver's license and ability to travel throughout Kentucky. Reliable transportation is required. Success in This Role The Director of Outreach will be expected to: Grow qualified referrals and admissions across the Eldercare network. Develop and maintain strong referral partnerships. Increase occupancy across all 15 personal care homes. Deliver an exceptional admissions experience for residents, families, and referral partners. Represent Eldercare with professionalism, integrity, and compassion. Pay:

$60,000.00 - $73,000.00 per year
