The Facilitator, under the guidance of the Program Supervisor, will be an integral part of a team that provides social and emotional support for the families of clients referred for intensive mental health services. This person will maintain highly coordinated communication with all family members, system partners, and other individuals involved in the life of the child and family. This person will also guide the development of the family plan of care, crisis and safety plans, and facilitate the monthly Child and Family Team meetings.

Job Duties and Responsibilities:

Provide Wraparound services to children and families, including service coordination and implementation, facilitate Child and Family Team (CFT) meetings, linkage services, 24/7 crisis response, and more. Cultivate relationships with county social workers, school personnel, mental health professionals, and others relevant to cases. Provide overall guidance and direction to ensure that the Family voice is heard and that the Plan of Care is followed. Provide psychoeducation and support to parents as they are learning about their child's needs and problems Initiate and complete all documentation including strength summary, plans of care, crisis and safety plans, and progress notes. Promote Wraparound principles like family-centered practice, flexible service delivery, parent/family professional partnership, family decision making, collaboration across systems, community service delivery, inclusion of family and community resources, and strength-based practices. Collaborate frequently with Wraparound team members, supervisors, and additional Olive Crest staff in a variety of departments. Willingness to provide after-hours and weekend support as needed, primarily over the phone, to families experiencing crirs. All other duties as assigned.

Required Qualifications:

A Bachelor's degree in Social Sciences, Social Work, Psychology or related field and one-year experience working with high-risk you OR an Associate's degree in a related field plus two (2) years' experience working with high-risk youth. Bi-lingual, English and Spanish. Must obtain and maintain record clearance free from criminal convictions. Negative Mantoux PPD Test/chest x-ray not more than one year prior to commencing work and every three (3) years thereafter confirming employee is free of tuberculosis. Must have CPR and First Aid certification upon hire and maintained throughout employment. Must have and maintain through course of employment a valid California Driver License, clean driving record, and vehicle insurance in accordance with Olive Crest's auto insurance policy. Able to work both independently with minimal supervision and collaboratively as part of a team. Demonstrates flexibility, dependability, professionalism, strong communication (verbal and written) skills, and be highly organized. May spend significant time driving as part of regular duties. Comfortable with participating in after hours support, often via phone, to families experiencing crisis. Must be able to physically intervene with youth up to 18 years of age during a crisis. Must have a working knowledge of Microsoft Office Suite and trainable in use of Electronic Health Record. Must have a philosophy of support for abused children in line with values, mission and treatment philosophy of Olive Crest. Must maintain eligibility to participate in federal health care programs. Eligibility is checked monthly through the Federal, State, and County Sanctions lists.

PREFERRED QUALIFICATIONS

Familiarity with, and willingness to utilize, community resources. Comfortable building relationships with culturally diverse families. At least two (2) years full-time equivalent experience (paid or unpaid) in delivering services in the mental health field.
