Social Worker Reports to: Program Director, LCSW The Social Worker provides strengths-based case management and supportive services to individuals and families participating in Jesus Center housing and supportive service programs. This position maintains an assigned caseload, assesses participant needs, develops individualized service plans, coordinates community resources, responds to crises, and supports participants in achieving housing stability and greater independence. The Social Worker collaborates with program staff and community partners to address barriers that may include homelessness, behavioral health needs, substance use, medical conditions, disability, domestic violence, aging, and other challenges impacting stability and well-being. Essential Duties and Responsibilities Case Management & Participant Services Maintain an assigned caseload and prioritize services based on participant needs and program requirements. Schedule and conduct regular case management meetings with participants. Complete assessments and work collaboratively with participants to identify strengths, barriers, goals, and service needs. Develop, implement, and monitor individualized service and housing stability plans. Connect participants with appropriate housing, healthcare, behavioral health, substance use treatment, employment, benefits, and community resources. Utilize strengths-based, trauma-informed, person-centered, and solution-focused approaches. Support participants in addressing barriers to obtaining and maintaining stable housing. Monitor participant progress and adjust service plans as needs and circumstances change. Respond appropriately to crisis situations and coordinate with supervisors, community providers, or emergency services when necessary. Establish supportive professional relationships while maintaining appropriate boundaries. Documentation & Compliance Maintain complete, accurate, timely, and confidential participant records. Complete case notes, assessments, service plans, referrals, progress updates, and other required documentation. Effectively utilize the Homeless Management Information System (HMIS) and other required databases. Maintain participant confidentiality and comply with organizational privacy and information-sharing requirements. Prepare monthly reports and other program information as requested by the Program Director. Team Leadership & Collaboration Participate in case conferencing and multidisciplinary team meetings. Provide consultation and guidance to staff regarding case management practices, participant engagement, professional boundaries, and service coordination. Delegate appropriate responsibilities to Housing Coordinators and/or Resource Specialists and provide direction and feedback. Communicate significant participant concerns, safety issues, and program needs to the Program Director. Promote consistent, participant-centered case management practices across programs. Community Collaboration Develop and maintain productive relationships with community service providers and partner organizations. Coordinate services with housing, healthcare, behavioral health, substance use treatment, employment, benefits, legal, and other community resources. Attend community meetings, trainings, collaborative groups, and networking opportunities as assigned. Represent the Jesus Center professionally with community partners and stakeholders. Qualifications Education & Experience Bachelor's degree in Social Work (BSW) or a closely related human services field required; MSW preferred. Experience providing case management, housing services, homeless services, behavioral health, or other human services preferred. Experience working with individuals and families experiencing homelessness or significant barriers to stability strongly preferred. Knowledge of community resources and coordinated service systems preferred. Experience with HMIS or similar case management databases is desirable. Knowledge, Skills & Abilities Demonstrated commitment to confidentiality, professional boundaries, ethical practices, and sound judgment. Ability to build respectful and professional relationships with participants, coworkers, and community partners. Strong verbal and written communication skills. Ability to recognize concerns, gather relevant information, assess situations, and develop appropriate solutions. Strong critical-thinking, problem-solving, and decision-making skills. Ability to effectively engage individuals experiencing homelessness, behavioral health challenges, substance use disorders, medical conditions, disabilities, domestic violence, trauma, or other significant barriers. Ability to remain calm, professional, and solution-focused during crisis situations. Strong organizational skills with the ability to manage multiple priorities, meet deadlines, and maintain accurate records. Ability to work independently while recognizing when supervisory consultation is necessary. Demonstrated cultural humility and ability to provide respectful services to people from diverse backgrounds and circumstances. Other duties may be assigned as necessary to support program and organizational needs.
Pay:
$26.00 - $30.00 per hour
