Job Description

Social Worker Work Schedule:
Monday-Friday, 8:00 a.m.-5:00 p.m.
Assignment Duration:
Through October 31, 2026 (subject to adjustment) Responsibilities Care Management — 70% Collaborates with interdisciplinary teams to resolve psychosocial needs, including financial issues, transportation, and community support. Coordinates psychosocial services using community resources and support networks. Works with utilization management and community partners to identify members who may benefit from case management. Provides social work consultation, education, and advocacy for members and families. Analyzes assessments to determine needs and appropriate resources. Monitors services to ensure effective resource utilization. Evaluates and anticipates member needs, offering support while promoting independence. Provides referrals based on assessed needs. Partners with members, families, physicians, and providers to achieve care plan goals. Participates in proactive identification of members appropriate for care management. Initiates follow‑up care and develops individualized care plans (assessment, education, coordination). Accepts case management referrals and serves as a liaison to community agencies and State of California personnel. Maintains documentation and statistics demonstrating case management impact. Initiates case conferences as needed. Support Services — 25% Contributes to a mission‑driven culture emphasizing quality, customer service, consistency, dignity, and accountability. Supports departmental responsibilities and organizational goals. Develops and maintains a network of community resources and services. Other Duties — 5% Completes additional projects and responsibilities as assigned. Minimum Qualifications Master's degree in Social Work. 2 years of experience working with gerontology populations. Experience in behavioral health, health facilities, health plans, counseling, case management, home health, or hospice. Equivalent combinations of education and experience may be considered. Valid driver's license, reliable transportation, acceptable driving record, and auto insurance (required for roles with 50%+ travel). Preferred Qualifications Bilingual in English and one of the following: Arabic, Farsi, Chinese, Korean, Russian, Spanish, or Vietnamese. Knowledge & Abilities Build and maintain effective working relationships with leadership, staff, and external partners. Work independently and exercise sound judgment. Communicate clearly in writing and verbally. Work flexible hours, including evenings and weekends as needed. Strong organizational, analytical, and project‑management skills. Perform effectively in a fast‑paced environment. Manage multiple projects and identify collaboration opportunities. Motivate and lead teams and committees. Proficiency in Microsoft Office (Word, Outlook, Excel, PowerPoint) and related systems.
