In this vital role, you will provide comprehensive social services to outpatient dialysis patient and their families, supporting their emotional well-being, fostering resilience, and connecting them with essential resources. Your expertise will help empower our patients to achieve greater independence and quality of life. This position offers an exciting opportunity to make a meaningful difference while working in a collaborative and supportive environment. Duties Provide social service consultations and support to both hemodialysis and peritoneal dialysis patients in accordance with the various rules and regulations as well as the facility's policies and procedure. Develops and writes an accurate comprehensive psychosocial assessment and plan of care of the patients according to the center's policy. Participates in the patients' Care Plan Meetings with the dialysis team. Writes a quarterly or monthly progress notes reflecting the social treatment plan and progress of the patients. Provide counseling to patients and their families. Interprets social, psychological, emotional and family problems to attending physician and patient care team members. Provides consultation to staff members, community agencies and other persons and/or groups seeking guidance in efforts to solve the problems of the patients. Provides the dialysis staff educational in-services regarding the social and emotional concepts of illness and their effects on patients and families. Evaluates patients annually for vocational rehabilitation and provides referral as needed. Assist patients with insurance issues and maintain regular communication with the billing personnel. Takes active participation in the facility's Patients' Grievance resolution process. Requirements Current LCSW license or Masters degree in Social works with Associate social work certificate. One year clinical experience. Strong communication skills with the ability to build trust and rapport quickly. Ability to handle sensitive situations with professionalism, empathy, and confidentiality. Bilingual English/Spanish .
