Social Worker, Master Degree Social Worker Silicon Valley ADHC Milpitas, CA Job Details Full-time $34 - $40 an hour 1 hour ago Qualifications Long term care experience Research Psychology Social Work Full Job Description The Silicon Valley Adult Day HealthCare is a therapeutically oriented outpatient day program that provides health maintenance, rehabilitative services, socialization, case management, and caregiver support/education and day respite, in a congregate setting to geriatric participants who are functionally impaired and are often the frail elderly.
Duties:
Reporting to the Program Director, the Social Worker is a professional who works collaboratively with a team of licensed professionals, including Occupational Therapist, Speech Therapist, Physical Therapist, RN's and Dieticians, to coordinate Social Service planning and services that assists participants and their families in understanding and coping with emotional and social problems that affect their health status. Responsible for the provision of ADHC social services, including various aspects of assessment (initial assessment, quarterly note, periodic assessment, annual assessment, etc), care plan development and advocacy for each participant. Evaluates the potential clients as a total person; identifies the limitations which impede their daily living; monitors and reports case recording on a timely basis; participates/leads in care plan development and coordination of social/health services. Researches and coordinates community referrals. Conducts home assessments and outreach; monitors enrollment of clients; participates in interdisciplinary team meetings and provides consultation to therapists, families, and participants. Assists with the Director's day‐to‐day management of the Social Work Department, supervision of staff, audit preparations, and reporting (i.e. MSSR, Plan of Correction, etc).

Social Worker applicants need to satisfied "ONE of The Following Qualifications": 1. Has a master's degree in social worker from an accredited school. 2. Has a master's degree in psychology, gerontology or counseling from an accredited school and has one year of experience providing social services in the fields of aging, health or long-term care services. 3. Has a license by the California Board of Behavioral sciences. 4. Has a bachelor's degree in social work from an accredited school with two years experience providing social services in one or more of the field of aging, health or long-term care.
