This position involves working collaboratively with a multidisciplinary team to assess, plan, and implement individualized care plans that address the physical, emotional, and social needs of elderly participants. The ideal candidate will demonstrate a strong commitment to enhancing the quality of life for seniors while ensuring their safety and well-being. Duties Conduct thorough assessments of participants' social, emotional, and environmental needs. Develop and implement individualized care plans in collaboration with the healthcare team. Provide counseling and support to participants and their families regarding care options and resources. Facilitate access to community resources, services, and support systems. Monitor participant progress and adjust care plans as necessary. Advocate for participants' rights and needs within the healthcare system. Participate in interdisciplinary team meetings to discuss participant care and progress. Maintain accurate documentation of assessments, interventions, and outcomes in compliance with regulatory standards. Experience A Master's degree in Social Work (MSW) from an accredited institution is required. Current state licensure as a Social Worker is preferred. Experience working with elderly populations or in geriatric care settings is highly desirable. Strong interpersonal skills with the ability to communicate effectively with diverse populations. Knowledge of community resources available for seniors and their families. Proficiency in case management practices and documentation standards. Ability to work collaboratively within a multidisciplinary team environment. This position requires a compassionate individual who is dedicated to improving the lives of elderly participants through effective social work practices.
