Position Summary
This single, unified role manages a diverse caseload spanning critical developmental milestones: early childhood school entry (Kindergarten), foundational primary education (Grades 1-2), middle school adaptation (Grade 7), and high school secondary transition planning (Grade 8). The Case Manager oversees the development, documentation, and compliance of Individualized Education Programs (IEPs) in accordance with the Connecticut State Department of Education (CSDE) and IDEA regulations.
Core Responsibilities
Across All GradesIEP Compliance:
Lead Planning and Placement Team (PPT) meetings, author legally compliant IEPs, and monitor goal progress.
Data-Driven Practice:
Conduct formal educational assessments, perform behavioral observations, and analyze data to adjust instructional support.
Collaboration:
Act as the primary liaison between general education teachers, families, and specialized therapists (SLPs, OTs, PTs).
Instructional Support:
Design and implement individualized accommodations, modifications, and positive behavior support plans.
Grade-Specific Caseload Responsibilities
Spring Kindergarten Case ManagementFocus:
Early childhood intervention and primary school transition.
Duties:
Guide families through their initial PPT meetings and establish early childhood indicators.
Duties:
Coordinate heavily with birth-to-three programs and speech/occupational therapists.
