PRIMARY PURPOSE

• Provides professional clinical and therapeutic social work services to acute senior behavioral health program for patients and their families in a manner consistent with the philosophy and objectives of the facility, and in compliance with regulatory requirements and best practice.

Leads and participates in the multidisciplinary treatment team process and develops and oversees the ongoing completion of the plan of care. Works collaboratively with members of the interdisciplinary health care team, initiates and drives discharge planning starting on admission, and provides the necessary patient and family education to meet the patient's overall care needs during admission and for discharge. The Social Services Clinician's main responsibilities include: completing and documenting the necessary screens and assessments and responds to needs identified. Assists in the development and delivery of the daily therapeutic program, providing psychosocial group/s as outlined in the daily program schedule. This role also provides individual and family therapeutic interventions as needed to address the psychosocial needs of the patient and facilitation of an effective discharge plan. This role serves as the main contact for interfacility and interagency probate related processes and collaboration between community agencies as necessary for care of the patient. The social services clinician leads and assists in the ongoing therapeutic milieu and patient interventions strategies to maintain a trauma informed person

• centered approach in the program.

The role collaborates with other individuals and departments when needed to support the case management process. The individual serving in this role will work primarily with and provide care to older adults and seniors, age 55 years and older, experiencing acute mental health and cognitive disorders in an acute behavioral health setting. Responsible to find the root cause of health care disparities

EDUCATION

• Master's degree in Social Work or Counseling from an accredited university.

EXPERIENCE

• Prior experience preferred working with the acute psychiatric population, preferably in an inpatient setting.

Prefer experience with case management, discharge planning, group leadership skills, crisis and behavior management, family therapy theory and practice. Experience in writing goals and Objectives which are measurable and treatment mobilities/interventions that support a successful discharge.

CERTIFICATE/LICENSE

• Current Kansas licensure (LCSW, LMSW).
