Job Description

Job Summary Woburn Rehabilitation and Nursing Center is seeking a dedicated and experienced Director of Social Work to join our dynamic team. As a 140-bed facility committed to providing exceptional resident care, we are looking for a compassionate leader to plan, develop, implement, and oversee social service programs that address the unique needs of our residents. The Director of Social Work will work closely with residents, families, staff, and interdisciplinary team members to promote quality of life, coordinate services, and ensure residents receive comprehensive social and emotional support. Responsibilities Plan, develop, organize, implement, evaluate, and direct the facility's social service programs. Build strong relationships between residents and staff to support individual resident needs. Identify resources and options that best address the physical, emotional, and social needs of residents. Maintain communication with families regarding changes in health, current goals, and care planning. Make appropriate referrals and obtain services from outside agencies and community resources. Provide or arrange necessary counseling and supportive services. Assist residents with financial and legal matters as appropriate. Participate as a member of the interdisciplinary team in reviewing facility policies and procedures. Support compliance with applicable state and federal regulations. Provide leadership and guidance to the social services team. Promote effective communication and collaboration among residents, families, staff, and outside agencies. Advocate for resident rights, dignity, independence, and quality of life. Qualifications Current LICSW licensure required. Clinical supervision and management experience preferred. Minimum of two years of experience working in a skilled nursing facility. Short-term rehabilitation and discharge planning experience required. Excellent communication, documentation, and assessment skills. Strong organizational and leadership abilities. Genuine concern for and interest in people. Ability to work effectively with residents, families, staff, and interdisciplinary team members. Knowledge of applicable social service practices and long-term care regulations.
