Job Summary We are seeking a compassionate and dedicated Licensed Social Worker to join our dynamic healthcare team. In this vital role, you will provide comprehensive social work services to patients and their families, supporting emotional well-being, facilitating access to community resources, and ensuring quality patient care. Your expertise will help guide individuals through complex health challenges, including end-of-life care, behavioral health issues, and recovery processes. Duties Conduct thorough patient assessments to identify social, emotional, and environmental needs, ensuring adherence to social work practice standards and clinical ethical guidelines. Develop personalized care plans that incorporate community resource coordination, discharge planning, and behavioral therapy strategies tailored to individual needs. Provide clinical counseling and psychotherapy, including individual and group therapy sessions focused on mental health counseling, addiction counseling, or behavioral health issues. Collaborate with interdisciplinary teams in clinical interprofessional collaboration to optimize patient outcomes and ensure holistic care delivery. Maintain accurate medical documentation and medical records in compliance with HIPAA regulations and ICD coding standards such as ICD-10 and ICD-9. Assist patients with navigating healthcare systems including Medicare, Medicaid, and other insurance programs while supporting discharge planning and post-acute care arrangements. Engage in crisis intervention when necessary, providing immediate support for patients experiencing mental health crises or behavioral emergencies. Requirements Valid licensure as a Licensed Social Worker (LSW or LCSW) with appropriate state certification. Proven experience working with diverse populations including geriatric patients, pediatric cases, individuals with developmental disabilities or autism spectrum disorder, and those requiring dementia or Alzheimer's care. Strong knowledge of medical terminology, clinical research methods, CPT coding, ICD coding systems, and EMR systems for efficient documentation. Demonstrated skills in crisis management, motivational interviewing, cognitive behavioral therapy (CBT), applied behavior analysis (ABA), and other evidence-based therapeutic modalities. Experience working within hospital settings, inpatient units, nursing homes, assisted living facilities, hospice & palliative medicine environments or home & community care settings. Familiarity with medical social work practices related to end-of-life care, post-acute care experience, acute care experience, and working with individuals from diverse cultural backgrounds. Ability to perform intake assessments accurately while adhering to social work practice standards and confidentiality requirements under HIPAA regulations.
