In this role, you'll play an important part in supporting the emotional and social well-being of skilled nursing residents by ensuring their individual needs are understood and addressed. You'll build meaningful relationships, collaborate with healthcare professionals, and help deliver person-centered care that promotes comfort, dignity, and quality of life.   • $63,877 / year Key Responsibilities Conduct psychosocial assessments and develop individualized care plans that support residents' emotional, social, and overall well-being.
Partner with residents, families, and interdisciplinary care teams to identify needs, establish goals, and coordinate services. Provide guidance, counseling, advocacy, and support to residents and their families throughout their healthcare journey. Connect residents with community resources, financial assistance programs, and specialized services to enhance quality of life. Coordinate discharge planning and care transitions to ensure a smooth and successful move to the next level of care. Advocate for residents' rights, preferences, and psychosocial needs while promoting dignity and person-centered care. Maintain accurate documentation and ensure compliance with federal, state, and local regulations, as well as organizational policies. Participate in quality improvement initiatives, team meetings, training programs, and collaborative efforts that support exceptional resident care. Skills, Knowledge and Expertise Required Qualifications Bachelor's degree from an accredited School of Social Work OR a bachelor's degree in a related human services field, such as Social Work, Sociology, Psychology, Rehabilitation Counseling, Special Education, or a closely related discipline. Preferred Qualifications One (1) year of supervised social work experience in a healthcare setting working directly with individuals. Experience working with geriatric, skilled nursing, long-term care, or rehabilitation populations. Strong communication, advocacy, care coordination, and problem-solving skills.
