Flex Day Program:
The Flex Day program allows employees the flexibility to take one day off within a two-week pay period by adjusting their work schedule. Employees remain responsible for completing their full scheduled hours for the pay period by working additional time on other days. Meaningful, relationship-based work supporting individuals in their homes and communities Mileage reimbursement for required home visit and assessment travel Supportive team environment with autonomy and professional respect Position Summary The Social Worker / Supports Coordinator provides MI Choice Waiver case management services, including assessments, service coordination, monitoring, and documentation. This role requires independent time management, professional judgment, and consistent performance to maintain hybrid work flexibility. Basic Job Duties Complete accurate and timely documentation in compliance with program, regulatory, and organizational requirements Communicate effectively with office staff, support coordinators, healthcare providers, and community agencies Conduct required home visits and community-based assessments within the assigned service area Requirements Licensed Social Worker in the State of Michigan (LLBSW, LBSW, LLMSW, or LMSW) Understanding of confidentiality and HIPAA regulations Strong interpersonal, organizational, and communication skills Ability to work independently while meeting productivity and quality standards Preferred Qualifications One year or more of social work or case management experience Experience in home health care, waiver services, or community-based programs Knowledge of healthcare regulations and quality management principles Benefits Competitive salary based on license and experience $5,000 sign-on bonus Health, dental, and vision insurance Life Insurance/Short Term Disability/Critical Care/Accident/Hospital 401(k) with employer matching Paid time off (PTO) and paid holidays Mileage reimbursement for required travel
