As a compassionate and dedicated Social Worker/Care Companion, you will be an integral part of our Collaborative Care team at Carease Health
MI.
In this rewarding role, you will provide emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities. We are seeking a motivated and experienced professional to join our team, working approximately 16 hours per week, Monday-Friday, within regular 8-5 hours.
Key Responsibilities:
Visit designated facilities in accordance with the schedule set with your supervisor Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns Document visit notes and maintain detailed records of interactions and interventions after each visit Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric
NP Qualifications:
Associate Degree in Social Work, Psychology, Counseling, or another mental health-related field Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings Strong communication, documentation, and relationship-building skills Why Join Carease Health?
