Clinical Social Worker-Case Management Provides social service and discharge planning functions for the hospital. Provides resources for inpatients, outpatients, Emergency Department patients and support to the Case Management team for psychosocial needs. Maintains performance improvement activities with the department.

Current Social Worker license for the state of
MS Certifications:
Basic Life Support (BLS) within 60 days of hire Minimum Work Experience Minimum 3 years' work experience in similar field of social work. Required Skills Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action. Essential Functions Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives. Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions. Participates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan. Assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan. Collaborates with providers, nursing and ancillary staff. Engages in practice that comply with regulatory standards, patient safety and performance improvement initiatives.
