Social Worker As a Social Worker , you will play a critical role in providing emotional and social support to our residents and their families. Your responsibilities will include assessing residents' needs, developing care plans, and coordinating with healthcare professionals to ensure comprehensive care.
Social Worker Key Responsibilities:
Conduct assessments of residents' social, emotional, and psychological needs. Develop and implement individualized care plans in collaboration with the healthcare team. Provide counseling and support to residents and their families. Assist residents in accessing community resources and services. Advocate for residents' rights and well-being. Coordinate and participate in interdisciplinary team meetings. Maintain accurate and confidential records of residents' progress and care plans. Facilitate support groups and educational programs for residents and families.
Social Worker Qualifications:
Bachelor's degree in Social Work (BSW) or related field; Master's degree in Social Work (MSW) preferred. Valid state licensure or certification as a Social Worker in New Jersey. Experience working in a healthcare or long-term care setting is preferred. Strong interpersonal and communication skills. Ability to work collaboratively with a multidisciplinary team. Empathy, compassion, and a genuine commitment to improving the lives of residents. Excellent organizational and documentation skills.
