Admissions/Discharge Coordinator The Ohio Eastern Star Home We are seeking a compassionate and caring discharge planner to join us in our mission to ensure that our residents are living their very best lives. Key Job Responsibilities:

• Complete psychosocial history, mood, and behavioral assessments
• Meet with guests and families to assess discharge needs after rehab stay
• Identify potential barriers to discharge
• Meet with interdisciplinary team to determine recommendations to meet guest's needs after discharge
• Assist guest in navigating and understanding their insurance benefits, approval process, appeals, etc.
• Provide tours of Ohio Eastern Star Campus to guests/families
• Communicate regularly in-person and by phone with guest and families to provide support and answer questions throughout their stay
• Communicate discharge plans to appropriate staff and coordinate transitions to other areas of our campus
• Facilitate the discharge plan by making referrals for home health/assisted living/SNF, order necessary DME, arrange follow-up appointments, coordinate transportation, and provide any additional resource information that could benefit guest after discharge Preferred familiarity or experience with:
• Local services and providers: home health, SNFs, DME, etc.

• Insurance:

  Medicare, Medicaid, commercial, vendors/plans, prior authorization process
• PASRR and Medicaid LOC process
• Excellent communication skills and a team-oriented attitude
• Previous case management experience preferred.
