Foster Care Coordinator - Tuscarawas Co

Job Description

Passionate about foster kids and families! The Foster Care Coordinator cares for foster children and families by providing case management, family support, training, and communication. This position will be serving the Wooster, Ashland, Holmes and Tuscawaras areas.

Requirements :
Bachelor's degree in Social Work, or related field, and active Ohio Social Work license (LSW) preferred, or experience in a children's services agency. Two or more years working with children and families preferred. Foster care experience strongly preferred.
Responsibilities :
Adhere to CCHO's statement of faith and code of conduct. TEAM first. Assist with intakes, facilitating placements of foster youth in assigned homes. Conduct regular home visits, meeting the needs of foster youth and family. Provide excellent customer service to foster families, placing agencies, including but not limited to timely documentation, facilitate transportation, etc., as needed. Provide case management services to help youth remain n the home and/or community, according to the needs of the child. Provide and/or coordinate mental health services for foster children and/or family. Participate in team meetings, clinical supervision, peer/case review, training activities, recruitment activities, etc., as needed. Timely completes and submits all required clinical documentation on assigned youth in electronic health record system. Rotate on-call duties, which includes after-hours crisis management for all youth and handling referrals.
