Responsibilities:

Provide social work services to residents, including assessing their needs, developing care plans, and coordinating services to meet those needs Collaborate with interdisciplinary teams to ensure that residents receive comprehensive and individualized care Conduct individual and group therapy sessions to promote resident well-being and independence Develop and maintain relationships with residents, families, and external agencies to ensure seamless transitions and coordination of care Participate in quality improvement initiatives to improve resident outcomes and experiences Maintain accurate and detailed records of resident interactions and care plans Comply with all facility policies and procedures, as well as applicable state and federal regulations

Requirements:

Minimum 0 years of experience as a Social Worker in a healthcare setting Strong communication and interpersonal skills Ability to work effectively in a fast-paced environment and prioritize tasks Strong analytical and problem-solving skills
