ABOUT THE EARLY INTERVENTION ACCESS AND COORDINATION PLANNING GRANT
The Early Intervention Access and Coordination Planning Grant is designed with the goal to develop a coordinated and equitable early intervention system of care that increases access and improves outcomes for children with developmental concerns. To ensure that all families in our community receive timely, high-quality services through a collaborative, data-driven regional planning process.
ABOUT FAMILY SYSTEMS NAVIGATOR
The Family Systems Navigator connects families with young children (birth through age five) to community resources, medical care, and developmental screenings. They act as a central point of contact, guiding caregivers through complex systems of care to support early childhood development.
RESPONSIBILITIES
Family Intake & Support Answer incoming calls and emails from families seeking assistance Listen to family concerns and assess developmental or basic needs Provide compassionate, culturally sensitive care coordination Developmental Screening & Support Introduce and help parents complete screening tools such as Ages and Stages Questionnaire (

SE, ESQ, etc) Interpret screening cores and explain results to family Provide guidance related to developmental milestones to support screening results Resource & Referral Connection Match families with local medical providers, early intervention programs, or social supports Build and maintain an updated database of community resources Follow up with families to ensure a successful connection to needed services Collaboration & Outreach Partner with local pediatricians, childcare centers, and community agencies Represent Center for Early Success and the grant imitative at community health fairs, coalitions and network meetings Collect and maintain accurate client records and input data into tracking systems Perform other duties as assigned in support of the Center for Early Success mission and grant objectives. Relationship Building Build relationships with Pediatricians, Part C system, Head Start, Early Childhood Special Education, school district preschool program, and any other publicly funded preschool programs to facilitate collaboration and establish referral pathways. Build trusting, positive relationships with families and helps connect to community resources Serves as a liaison between families, providers, and community resources for families, and promote community resources, events and related activities to families. Maintain a strong understanding of early childhood issues and systems. Data Collection & Reporting Collect data and track progress including resources and referral trends Maintain accurate client records Responsible for the complete and accurate collection, entry, reporting and maintenance of data, records, and reports.
MINIMUM QUALIFICATIONS
Bachelor's degree in early childhood education, human services, or related field 3-5 years of experience in early childhood systems, early intervention, healthcare, education, or human services Advanced proficiency in Microsoft Word, Excel, PowerPoint, Outlook Strong understanding of typical child development milestones Strong knowledge of early intervention systems (Part C and Part B) Grant implementation experience Extensive experience with Early Childhood Education. Excellent interpersonal, written, and verbal communication skills Must be able to lift a minimum of 35 pounds Must have a valid VA driver's license and insured vehicle In person office position in

$51,000.00 - $68,000.00 per year
