Job Description

Provides psychosocial support and counseling to patients, families and caregivers; identifies and coordinates community resource needs; complete bereavement assessment and plan of care and perform bereavement tasks. Must have a Master of Social Work Degree accredited by the Council on Social Work Education.

Up to $45.00 per hour
